Registration Information and Forms 

Registration is open to Q.U.I.L.T.S. members only due to spacial constraints.  However, we are accepting quilts made by children 16 years old and under who are sponsored by a member.  The registration will be under the member’s name but in the Description section of the form please provide the child’s name, age, school and grade, relationship to member, and information about the quilt.

Please use a separate form for each entry and enclose a self-addressed stamped envelope (SASE) so that your stub(s) can be returned to you.  If you are entering more than one piece, as long as the registration forms are submitted together, you will only need one SASE.

There is a fee of $5 for each quilt you want judged. Please make your check payable to Q.U.I.L.T.S. and include it with your registration forms(s) and SASE.  

Read the form and instructions carefully, as there have been some changes from previous years. 

All registration forms should be received no later than August 27, 2010.
